Pre-requisites (Requirements to Prepare Before Applying Online)

 

Renewals Tax Registration (BIR):
BIR Form 1901 (Sole Prop.) or 1903 (Corp/Partnership).
Taxpayer Identification Number (TIN) (if you don’t have one).
Tax Type Questionnaire (to determine tax liabilities).
₱30 Documentary Stamp Tax (DST) payment receipt.
Scanned copies of DTI/SEC Cert, Valid ID, and Proof of Address (Lease Contract/Title).

Please secure the following documents/clearances before starting your online application:

Business Name Registration:

For new applicants :

    • DTI (Sole Proprietorship): Certificate of Business Name Registration.
    • SEC (Partnership/Corporation): Certificate of Registration, Articles of Incorporation/Partnership.
    • Cooperative Development Authority (CDA) for Cooperatives

. You must file a Cooperative Name Reservation Request Form (CNRRF) to check if your desired name is available. The name must not be identical or similar to existing cooperatives and must include the word “cooperative”.

Local Requirements:

  1. Barangay Business Clearance – Issued by the Punong Barangay
  2. Police Clearance – Issued by the Lezo Municipal Police Station. Please prepare GCASH as payment.
  3. Payment of Real Property Tax (Land & Building) – Treasurer’s Office
  4. Zoning Certification – Issued by the Municipal Planning and Development Coordinator (MPDC) Office
  5. Building Annual Inspection Report – Issued by the Municipal Engineering Office
  6. Mayor’s Clearance – Issued by the Office of the Municipal Mayor
  7. Medical Clearance / Sanitary Permit to Operate – Issued by the Sanitation Inspector or Municipal Health Officer (MHO)
  8. Waste Management Certification – Issued by the Municipal Environment and Natural Resources Officer (MENRO) Office

Step-by-Step Application / Renewal Process

  1. Business Owner or Authorized Representative
    Log in or register at the Online BPLS Portal
    Fill out the application form completely and accurately
    Upload scanned copies of all required pre-requisite documents/clearances
  2. Submit the original hard copies of all uploaded documents to the Business Permits and Licensing Office (BPLO) at the Municipal Hall.
  3. The BPLO will verify the submitted documents and application online.
  4. Concerned Endorsing Offices will review and confirm the application.
  5. The Municipal Treasury Office will assess the applicable fees and issue the Order of Payment (OP).
  6. Pay all assessed business regulatory fees at the Municipal Treasury Office.
  7. Pay the Fire Safety Inspection Certificate (FSIC) fee to the Lezo Bureau of Fire Protection (BFP)
    (Bring your Official Receipt (OR) from the LGU and/or TOP when paying at the Public Market area.)
  8. Claim your Fire Safety Inspection Certificate (FSIC) from the Lezo BFP
    (Bring your Official Receipt (OR) from the LGU and TOP when claiming at the Public Market area.)
  9. Once all requirements are complied with, the Business Permit will be released by the BPLO.

Important Reminders

  • Ensure all pre-requisite documents are complete and valid before applying online to avoid delays.
  • Business permits must be renewed every year on or before January 31.
  • For Fire Safety Inspection Certificate (FSIC), please coordinate directly with the Lezo Bureau of Fire Protection (BFP).
  • If you encounter any technical issues with the online portal, kindly visit the BPLO at the Municipal Hall for assistance.
